School Photo Authorization Form

Dear parents,

The school has contracted with The Photo Guys to do the photography at your child(ren)'s school this year. The school does not allow us to share student contact information without your permission. To guarantee that the photos remain confidential we propose that The Photo Guys be allowed to email/text you directly with the details that will allow you to connect to their website and view the photos of your child(ren). The Photo Guys have signed a confidentiality agreement ensuring that they will use your contact information only for this exclusive purpose.

Please let us know below whether or not you authorize The Photo Guys to contact you only for the following:

Picture Day reminder 1 week prior to picture day
Picture Day reminder 1 day prior to picture day
Notification when pictures are ready to View & Order
Notification before Free Shipping ends 7 days after picture day
Notification of upcoming Holiday Specials

If you would like to opt-in to ALL digital communications, please submit this form by picture day. Forms submitted after picture may not be included on time. Please call or email us to access your child(ren)'s portrait gallery.
